The BCAPOP Relapse Prevention Plan Cards are part of the BCAPOP Perinatal Substance Use Toolkit. They serve as a conversation starter for pregnancy outreach workers to talk with participants who have a history of substance use and a goal of reduced use or abstinence about relapse prevention planning.
